CALD Courses |

Are provided free of charge for staff working for primary (PHO and GP practices) and secondary care services funded by WDHB, ADHB, CMDHB and staff working for NDSA |
| B. |
Course Enrolment and Approval
It is your responsibility to: |
| B1. |
Notify your manager that he/she will be receiving an email request from CALD Admin to approve your course enrolment and he/she needs to approve your enrolment within ten (10) days from receipt of email, otherwise your enrolment will be deleted |
| B2. |
Ensure that manager's name and email address on the enrolment form is correct |
| B3. |
For staff working for ADHB, WDHB, CMDHB, NDSA and PHOs (not GP practices): |

B3.1 |
provides a work-related email of your approving manager eg. nn@waitematadhb.govt.nz or nn@adhb.govt.nz or nn@cmdhb.govt.nz or nn@middlemore.co.nz or nn@ndsa.nz or nn@procare.co.nz |

B3.2 |
notify cald@waitematadhb.govt.nz with correct manager's information within (ten) 10 days from enrolment date |

For staff working for GP Practices: (your Practice Manager is the approving manager) |

B4.1 |
Inform your Practice Manager to approve your enrolment within 10 days upon receipt of email |
| B5. |
You can print a copy of the Course Enrolment Approval Form for your manager to approve manually and fax to CALD Admin. |
| C. |
User Registration |
| C1. |
For staff working for ADHB, WDHB, CMDHB, NDSA and PHOs (not GP practices): |

C1.1 |
You must provide a work-related email on the user registration form |
| C2. |
For staff working for GP Practices: |
|
C2.1 |
You are allowed to register with non-work related email on the user registration form (however please note that course enrolments are still subject to approval from your practice manager) |
| D. |
Course Enrolment Confirmation Process
CALD Admin |
| D1. |
Can only confirm your enrolment after the manager has submitted his/her approval |
| D2. |
Can only confirm your enrolment on Monday to Friday 8.30am to 5pm excluding public holidays and weekends |
| D3. |
You will receive email notification confirming your course enrolment been approved or declined unless your preferred notification is by fax |
| E. |
Abuse of CALD Registration System, Course Enrolment, CALD Forum rules
CALD Admin has the discretion and authority to |
| E1. |
Decline or delete your enrolment if your approval has not been received within ten (10) days of enrolment and if enrolment information provided is in compliance with B3.1 |
| E2. |
Decline or delete your user registration if you have not complied with C1 |
| E3. |
Delete your registration, or any approved course enrolments or membership to the online CALD forum if you are found to have abuse the system or copyrights |
| F. |
Re-Register or Re-Enrolment |
| F1. |
You can re-enrol or re-register if your registration or enrolment has been deleted or declined by CALD Admin because of delayed approval or because of incorrect email address |
| F2. |
You will be required to comply with terms and conditions A, B, C when re-registering or re-enrolling. |
| G. |
Late Course Cancellation Fee |
| G1. |
This applies only to classroom course enrolments |
| G2. |
CALD admin has rights to charge a late cancellation fee of $100 per enrolment for all classroom-course cancellations or re-bookings received one week or less from the course date. The fee will also be applied to all no-shows on the day of the classroom delivered course.This fee is only waived if a staff member is sick and they or their manager ring us on the day of the course to inform us of this. |

Online Courses |
| H1. |
Online study expires after you receive your Certificate of Completion or expire 60 days from date of approval if course is partially completed or not undertaken. |
| H2. |
No fee will incur for partially completed or unattended online study at this stage |
| H3. |
You can enrol for the same online course up to 10 times only |
